The CAT Application Forms are available in all Axis Bank branches in major cities from September till october for Rs.1400/- (Rs. 650/- for SC/ST candidates).

A candidate only has to buy one form and should not apply multiple times. He can send the completed CAT 2010 form along with required documents to concerned IIM before September 5th or can personally handle the application to the office.
